POLICIES

1.Unacceptable specimens

  • Missing Requisition Form: All specimens must be accompanied by the requisition form enclosed in the mailable kit.
  • No payment: Payment must be enclosed with the specimens or prepaid on the website unless there is an account contract signed by the referring practitioner authorizing us to bill them. We accept checks, Visa, MasterCard, and Discover.
  • Insufficient Samples: Samples that lack sufficient fecal material cannot be diagnosed. Blood slides that are stuck together or swabs that are not correctly inserted into the test tube are also inadequate. If a sample is deemed inadequate, repeat samples are recommended. Patients are only responsible for the cost of the replacement kit and shipping. The test itself, which has already been paid for, does not need to be paid for again.
  • Discarding of Specimens: Specimens are discarded after examination or if deemed inadequate unless previous arrangements have been made.

2. Reporting of Test Results

  • Test results will be available to the practitioner 2-5 business days after receipt of specimens for all tests except the Swab Culture and H. pylori test. The results of the Swab Culture and H. pylori test will be available 2-3 weeks after receipt of specimens.

3.Treatment Recommendations

  • Recommendations for herbal or allopathic treatment will be shared with referring practitioners upon request.

4.Courtesy Testing

  • Referring practitioners receive complimentary testing after at least 3 referrals. Contact PCI for additional information.

5.Insurance Claims

  • PCI does not bill insurance companies directly. Patients may request an invoice by stating their request on the requisition form or, if ordering online, print one directly after purchase. PCI is not a Medicare provider. CPT codes are provided next to each test or product for patients to verify which products or tests their insurance companies will cover.

6.Payments

  • Payment is required in advance before any testing is done or any product is shipped, unless the practitioner overseeing the tests has a valid account contract authorizing us to bill them. PCI accepts checks and the following credit cards: Visa, MasterCard, Discover. All returned checks will be charged a $25.00 fee.

7.Refusal of Service

  • PCI reserves the right to refuse service to any individual for any reason.

8.Refunds and Cancellations

  • Test Orders: Orders for tests may be canceled within 90 days of the purchase date for a full refund. Testing packages and shipping charges once shipped are non-refundable.
  • Product Orders: Orders for products are refundable within 30 days of the purchase date. Testing packages and shipping charges once shipped are non-refundable, and return shipping is the customer's responsibility. For a full refund, products must be returned in their original packaging, unopened, and sealed. Any packaging that is damaged, opened, or labels covered will incur a 25% repackaging fee. No refund will be issued if the seal of the cap or lid on any supplements has been broken.

9.Shipping Labels

  • The shipment of samples is the responsibility of the patient. We do not provide return labels. Please ensure you arrange and cover all shipping costs to avoid delays.
